Paragon's STOMP II Medical Pack (56k Warning!)

It was actually a couple of weeks ago that I said I’d post my STOMP II pack, although it took me a lot longer than I expected to transfer everything over from my Excel file. My photographic skills suck, although I tend to be pretty OCD about that kind of thing, so the pictures took me a lot longer than I hoped they would. I did notice today there are a few things that I missed in the process of organizing/reorganizing everything, but I’ll try to catch those on the next pass.

One of the things that I thought would be helpful was to incorporate hyperlinks into all the item descriptions. While hopefully this will be useful to someone, it definitely turned into a major PITA, and there were more than a few times that I had pretty much decided to punt. I started to get lazy, so the links aren’t necessarily the cheapest ones on the internet, but provide further descriptions of many of the items. I’m also sure it won’t take very long for a bunch of the links to become 404 errors, but I’ll try to keep things updated as they change. Then again, Google probably isn’t going belly-up anytime soon, so you can always try that if a link is dead.

I’ve always enjoyed looking at pictures of everyone’s stuff, so I’ve included plenty in this post. If you didn’t heed the dialup warning, you’re pretty much screwed – you might as well go buy a case of beer and check back in a few days. If there’s something that I missed that you want to see (unlikely, considering that I exceed the thread limit of 60,000 characters by 50% and had to spread this out over two posts) let me know and I’ll do my best to add it.

As you work your way through the pictures and item descriptions, you’ll probably have a few questions:

1). No, I don’t plan on carrying this thing around on my back if I need to bug out on foot. Although I easily could if this were all I needed to carry (it only weighs about half as much as my BoB) this is simply my attempt to centrally organize a home-based, ALS/BLS FAK that I could throw into the car on short notice. I have several other FAK modules that I take to the range, keep stored in my vehicles, my BoB, and my GHB.

2). Although I’m familiar with the use of everything shown here, I don’t work in the medical field, nor do I ever plan to, and I am certainly not certified to use everything that I have. Hell, several of the items in my pack are even off limits to working paramedics without an online order from a physician. Many of the more advanced items shown I’ve only used on medical mannequins, and would likely never use on a living/breathing person except for in a PAW scenario. I feel that my current training is adequate and I certainly plan to continue to learn as much as I can and practice my skills, although several of the prescription items are for my own personal use only, while others are only for my immediate family, or very, very close friends, and only then in a PAW scenario where qualified medical assistance is unavailable. 3). No, I certainly don’t drive around looking for traffic accidents, checking under bridges for crack addicts and the homeless, or monitor police scanners to practice needle decompressions on GSW victims -– go back and read #2 again until this point is clear.

4). As I’ve mentioned before, there are a bunch of physicians and surgeons that live in my neighborhood, although of the ones that I hang out with, only one (an ER surgeon) maintains anything close to a comprehensive medical kit in his car. In a PAW or other natural disaster, my STOMP II could realistically become the only viable source of medical supplies and equipment for myself and my neighbors.

While I feel this FAK is fairly comprehensive, there are a few more things that I would like to add eventually. I don’t current have a pulse oximeter (having never received training in the use of one) but I feel this may be of some benefit in the future. I would also like to add an AED, and perhaps a D cylinder (415 liter) oxygen cylinder.

For the amount of money you have invested, I'm surprised you don't have an AED! You'd want one around if you use the epi-pens, and are more likely to use the AED than 50% of the gear in that pack.)

Nice kit, though! I've got most of the same stuff lying around at home, but not so neatly packed an organized. Plus, most of the stuff are things I use daily, so I don't bother with the burn pads and such. The tube and scope kit is a nice thing to have... I also have a nice set of angled forceps designed to grab onto airway obstructions.

There's actually quite a bit of Kerlix in there (counting the PriMed) - 14 large rolls (4.5" x 4.1 yds) in all, plus at least 70 or more 4" x 4" gauze pads/sponges that I remember off hand. I actually have another 35-40 rolls of large Kerlix and several hundred pads/sponges at home, but didn't feel that I needed them in the STOMP II.

The OB kit is indeed a bit of a stretch, but is something that features a bunch of relatively specialized items, is small and lightweight, and only cost around ten bucks. I’ve shown it more as a PAW reference, although there would be a bunch of real life day-to-day scenarios that I can think of where it would not make the trip if I were using the pack away from home, such as in the wilderness or on a camping trip.

Cough drops are located in the unitized medications bag (4 packs of Soprocaine® Throat Lozenges, 2/Pkg.). Like you, I've also read several less than favorable articles regarding the current use of Ipecac, although under certain circumstances (many poisions contraindicate the inducing of vomiting) it can be the difference in certain poisonings. Obviously great care regarding asphyxiation must be exercised, and I would only consider using the Ipecac if specifically directed to by a Poison Control Center. Activated charcoal is located in the Bite & Sting kit.

The hyperlinks are not necessarily meant to be the source of a particular item, merely one of many online links available that further described an item. With regard to my RX items, everything has been acquired locally, legitimately, and face-to-face. With regard to the opiates in particular, I do indeed have good relationships with several physicians that know me very well (as well as each other) and appreciate my commitment to training and preparedness, as well as the fact that I have no history of substance abuse. As I indicated, the Rx items that I have are for my own personal use, and only then in time of true need.

No, both are 0.3 mg each. The pediatric dose Epi-Jr. might be something to consider, although I don’t have any small children, nor do any of my neighbors for that matter.

-As you said, O2 could be usefull to add. Get a 1l. cylinder if you don't have enough spare room.

I'm sure you're always open to suggestions for improvement. It is, however, very difficult to find room for improvement with this kit.
The only things that jumped out at me are in the meds. That's an occupational hazard :
1) Stocking more than one amp of Narcan, as one amp is often insufficient.
2) Anti-emetics. I saw that you have phenergan tablets, which are valuable because they have other antihistamine effects. However, I've always found oral anti-emetic medications to be retarded and useless if you're vomiting, which is pretty obvious. You can get a suppository form of Phenergan (Promethazine HCl), and phenergan can also be injected intramuscularly. You can also get oral disintegrating tablets of Zofran (Odansetron HCl), which has fewer negative effects than Phenergan and I believe is also cheaper. Both Phenergan and Zofran can be given IV if this is within your capabilities. Dramamine may also be beneficial to have in the OTC kit. All of these anti-emetics work through different means.
3) Ibuprofen stocked in both OTC and Rx containers. As I'm sure you know, the only difference between the OTC and Rx tablets is the mg dose, so four OTC tablets equal one Rx tablet. Unless you have nothing else to stock there, this may or may not be considered wasted space in the Rx container. The 200mg OTC tablets provide more flexibility, as it's easier to give more of them than it is to break or cut an 800mg tablet.

4) TAPE! I may have missed it, but it looked like you have a large quantity of Durapore silk tape but nothing else. Tape allergies are very common, and even the hypo-allergenic Durapore you have is very rough in some people's skin and causes a rash because the adhesive is so tenacious. Switch a couple of those out for a roll each of paper tape (3M Transpore and Micropore, for example). Nexcare also makes a paper tape that is very gentle.
5) Celox is better than QuikClot, as it does not cause burns.

I really like that pouch you're using for the assorted bandages. Where did you get it?

I don’t disagree with you (and painiac) on the Narcan. I will probably add a couple more of the 0.4mg/mL amps, or at least one of the 1mg/mL amps. Until then the 2.5mL bottle of Stadol NS (Butorphanol Tartrate) should deliver the equivalent of 15 doses of Narcan, no?

The doc that originally wrote my script for the morphine didn’t like the idea of fentanyl at all (perhaps due to the chance for OD if someone didn’t realize dosages were in mcg versus mg). I realize fentanyl is ~80 times more potent than morphine with regard to quantity injected, although my understanding has always been that both offer identical analgesic effect. Is this not true?

This article seems to provide some pretty good information regarding the use of fentanyl in a prehospital environment.

I’ve seen the RES-Q-VAC before and thought it was pretty nice, although I guess that I can’t really picture how/where I’d ever use it. Are there common uses beyond oral, nasal, and wound suction? Between the 60cc and bulb syringes that I currently have in the kit, are there things this device would do that I can’t do now, or would this simply make the task faster/easier?

Yeah, I was pretty stoked when I found that. I had spent a few days talking with different people and researching steths and had pretty much decided the Littmann Master Classic II was the cat’s ass – being able to get one in all black made my decision easy.

Other than a ~2:1 weight-for-weight potency in favor of Benadryl, I was under the impression that dimenhydrinate (Dramamine) must dissociate into diphenhydramine (Benadryl) in the body before it becomes active. Doesn’t that suggest that diphenhydramine produces more pronounced effects (2:1 dosage) and much sooner (no dossociation)?

I’ve always kept several 800mg tablets in my FAK’s out of habit since I hardly ever take a lower dose than that. The 200mg caplets are really on hand more for others than myself, although you’re absolutely correct, as I can’t think of a single legitimate reason to carry both. I’ll drop the 800mg from the Rx compartment and add something else in its place.

Isn’t that kind of like saying that Coke is better than Pepsi? All of the QuikClot that I have is the newer (105°F) formulation, so burns are not an issue. Celox does seem to be a good product as well, although I do have the Hemcon in there if a PT is partial to seafood.

The bandage pouch is something that I bought off eBay several years ago, so I'm not sure to suggest looking for one other than that. I have an additional one that is very similar (bright red) that is the basis of my GHB/BoB FAK.

Thanks for the fentanyl-article. Good read. I would like to point out this:

The medical literature, coupled with field experiences, clearly demonstrates the superiority of fentanyl as an analgesic medication for use in the EMS environment. Fentanyl does not affect hemodynamics, oxygen saturation or GCS when used appropriately. It is fast-acting, short-acting and causes less histamine release than other narcotics. Traditionally, its use has been limited to critical care ambulances or air medical services, but many leaders in EMS are advocating that fentanyl become the primary narcotic analgesic in the field, because it causes less hypotension, respiratory sedation and mental status depression than morphine or meperidine. Also, its short duration of action would appear to be ideal for agencies that still have difficulty with emergency physicians who chastise use of narcotics that may mask symptoms of pain and hide illness or injury despite the overwhelming scientific opinion in the peer-reviewed medical literature that this is a myth.

Over here EMS only uses fentanyl for severe pain management. Morphine is now only used for cardiac astma as a vasodilator. Also, I like the way of calculating dosages for fentanyl (1 mcg/kg) and that it comes in 100 mcg/2ml ampules.



A suction device could be improvised with an 60ml syringe with NPA attached, but a res-q-vac is just much quicker and easier. In an aid-bag such as yours, it would be a great addition.

It's a necessity in a code. When you're doing CPR and rescue breathing a lot of vomit comes at you... filling the airway, and spraying on you. Suction:

1. Keeps more vomit off of you.
2. Cleans out the airway so you can see to tube them.
3. Helps reduce the amount of crap the patient aspirates. They'll drown in their own vomit and blood.

The patient will vomit, and bleed. With all that crap in there, your fantastic blade kit will be useless, because you'll end up having to tube them blindly.

Also, those little hand-operated vacs are the shit. They actually work better than most of the battery operated portable suction units.

I agree, to a point. 3M Micropore tape IS useless... but for light duty first aid, 2" paper tape is great. It's easy to apply and easy on the skin. We use it exclusively here at work (a wound care center). We have silk tape, but we deal with bandages that are changed daily, so silk is over kill.

Plus, whenever possibly we don't apply the tape directly to the skin. We wrap the wound with gauze, then tape the loose end of the gauze to the gauze already wrapped. That way there's no skin contact with the tape.

Also if this is meant as a more long term care kit then IV/Oral steroids might be in order. You can even get them in premade blister pack for idiot proofing. I did not see any IV/IM antibiotics these would be really usefull if you have some that has a really severe infection/Septic shock. Looks really dam good, on par it would seem to even the Doc bag.

Most IV/IM steroids and antibiotics require refrigeration, so I'll probably just stick with the oral AB's that I currently have. Deltasone (Prednisone) tablets on the other hand are an adrenocortical steroid that may be something to consider putting in my kit.

I second this. If you are dealing with a serious infection, you are really going to want some kind of IV / IM antibiotic. Ceftriaxone and ampicillin/sulbactam would be good choices because they don't require refrigeration. They come as a dry powder that you have to reconstitute with NS or some other compatible liquid.

I meant getting it as a IV IM drug not as oral but that would also be good. I just like the idea of controlling pain but also being able to get myself out of the shit I am in with a clear head.

Stadol is an opioid analgesic with a potency about 5x that of morphine, and should be used as such. It is an opioid agonist AND an antagonist, but in my opinion except in the most extreme circumstances it should not be used exclusively for its antagonist effect in somebody who is overdosed on opioids because it can have its own additive effect. Its antagonist effect is really only particularly useful in reducing dependency on itself.

Isn’t that kind of like saying that Coke is better than Pepsi? All of the QuikClot that I have is the newer (105°F) formulation, so burns are not an issue. Celox does seem to be a good product as well, although I do have the Hemcon in there if a PT is partial to seafood.

The bandage pouch is something that I bought off eBay several years ago, so I'm not sure to suggest looking for one other than that. I have an additional one that is very similar (bright red) that is the basis of my GHB/BoB FAK.

Toradol (oral) appears that it should only be initiated with IV or IM dosing of ketorolac tromethamine first, so that would limit its attractiveness for me.

Yes, get fentanyl if you can. But do not forget that toradol is superb. I've seen it work in many cases (post-op patients) when morphine was ineffective. Toradol is a powerful NSAID for short-term use (up to 5 days only). The problem with opioid analgesics, aside from all of their dangerous side-effects, is that they do not last very long. You need toradol, it is very effective when given IM. With just a little training, IM injections are not difficult. Just never give them in the ass because there's a danger of jabbing the sciatic nerve, causing permanent damage. The deltoid is ideal, and the vastus lateralus (middle third of the outer thigh) is a good injection site as well.

Most IV/IM steroids and antibiotics require refrigeration, so I'll probably just stick with the oral AB's that I currently have.

Most IV antibiotics are available as a vacuum-packed powder in a sealed vial that is reconstituted with saline or sterile water. Shelf life is very long with these.
The ADD-Vantage vial is a wonderful system for antibiotics, too, but won't be as cheap. You pop the caps, screw the vial of powder directly onto the special bag of IV fluid, pop the stopper into the bag, and reconstitute the powder right in the bag.
